World Menopause Day 2018 Promotion – Terms and Conditions

Terms and Conditions:

1. Promotion commences at 8.30am on the 18^th of October 2018 and closes at 5.30pm Friday 9^th of November 2018.
2. Participating in this promotion constitutes acceptance of these terms and conditions
3. This promotion is run by the Australian Menopause Centre Pty Ltd ABN 25 123 859 861 and is open to all Australian residents only
4. All staff of the Australian Menopause Centre are not eligible to participate in this promotion
5. To enter the promotion you must advise the relevant AMC staff that you would like to participate by mentioning World Menopause Day 2018
6. Gift vouchers are subject to the Gift Voucher Terms & Conditions (T&Cs), which form part of the contract of sale and use for this gift voucher as outlined by Coles Group Limited
7. Existing Patient is defined as any patient on our Treatment program that has paid for at least a 2-month treatment period
8. New patient is defined as any patient that has never been treated by our doctors nor has been on a treatment program with the Australian Menopause Centre previously
9. For existing patients: Gift Cards will be sent out only when the new patient joins the program, pays for the first 2 months of the program and does not cancel before the end of the 2 months of the treatment program
10. For new patients: Gift card will be sent out only after you join the program, pay for the first 2 months of the program and you do not cancel before the first 2 months are over
11. Gift cards cannot be redeemed for cash nor can it be used for the cost of the treatment program with the Australian Menopause Centre
12. Gift cards will be posted within 2 weeks of the qualifying date by Australia post
